Referral procedures
Referrals can be made by any health care professional with the consent of the GP or hospital consultant involved.
Patients and carers can self refer within six months of discharge otherwise re-referral by the Primary Care Team is required.
How to refer:
• Complete a Hospice Referral Form (this can be downloaded at the bottom of this page) or obtained by phone on 0141 429 9823 • Referrals can be made by post at the address below • Referrals can be made by fax: 0141 429 8406 • Fax referrals can be accepted provided they adhere to NHS MEL Guidelines (1997). • All referrals should be accompanied by relevant and recent clinical information on diagnosis, stage of illness, treatments and medication.
What happens next?
• The referrer will be informed of receipt of the referral within 2 working days of the referral being received • The clinical team will make a decision regarding acceptance of the referral and communicate this to the referrer within 2 working days • Access into the Hospice is access to all services. On acceptance of the referral to any of the hospice services a member of the multi-disciplinary team will contact the patient to arrange either admission to the in-patient unit or assessment at home dependant on the service allocated/ requested.
